Non-Diffractive Extended Depth of Focus

Vivity® IOL:
Extended Vision Without the Halos

The Vivity® IOL by Alcon works differently than traditional multifocal lenses. Instead of splitting light into separate focus points, it gently stretches your focus to give you a wide range of clear vision with fewer visual side effects like halos and glare. Our cataract surgeons help patients across the greater NY/CT region decide if this lens is the right choice for their vision goals and daily life.

Best For: Clear Vision With Minimal Visual Disturbances

Unlike traditional multifocal IOLs that use rings etched into the lens to split light into multiple focus points, the Vivity lens has a smooth surface with no rings at all. This is important because splitting light is the main reason older multifocal lenses can cause noticeable halos and glare, especially at night. The Vivity lens directs a greater share of incoming light toward forming a clear image than diffractive multifocal lenses, which means better contrast than those multifocal designs.

The Vivity IOL creates its extended range of vision through two smooth elements on the front of the lens. These elements gently stretch and shift light as it passes through, turning a single point of focus into a longer, continuous zone of clarity. Instead of jumping between separate focus points, your vision feels smooth and natural across a wide range of distances.

Important to know: While the Vivity offers better contrast than traditional multifocal lenses, clinical testing shows some reduction in contrast sensitivity compared to a standard monofocal lens, particularly in dim lighting. Your surgeon will discuss whether this tradeoff makes sense for your vision needs.

Who Is a Good Candidate?

Patients who are happiest with the Vivity IOL tend to spend a lot of their day on activities at arm's length or farther, and they value clear nighttime vision even if it means using reading glasses occasionally.

  • Active lifestyles focused on distance and mid-range vision. This includes driving, sports, cooking, computer work, golf, and outdoor activities where clear vision at arm's length and beyond matters most.
  • Patients who have had LASIK or PRK. The Vivity's smooth design works well even with mild changes to the cornea from past vision correction surgery. In a small cohort of 20 post-LASIK patients from the Vivity Registry Study, 70 percent landed within ±0.50 D of their target prescription, and 81.3 percent did not need glasses for near tasks.
  • Patients with mild, stable glaucoma. Because the Vivity does not split light, its contrast sensitivity is better than traditional multifocal lenses. However, like all EDOF lenses, it does show some contrast reduction compared to a standard monofocal lens. For patients with mild, stable glaucoma, this tradeoff is generally acceptable. Studies show the Vivity provides noticeably better mid-range and near vision than a standard single-focus lens, with very few bothersome visual side effects.
  • Patients worried about halos and glare. The Vivity produces visual side effects much closer to a standard single-focus lens than other premium options, which can be very reassuring if you are sensitive to light or have been told that a trifocal lens may not be ideal for you.
May not be the best fit? If you need sharp up-close vision for reading or crafts throughout the day, a trifocal IOL may serve you better. Patients with moderate-to-advanced glaucoma should discuss whether a monofocal lens may be a safer choice. Patients with a history of multiple corneal surgeries, significant dry eye, or retinal conditions that affect central vision should talk with our cataract surgeons about other lens options. Not all patients are candidates for premium lenses; your surgeon will evaluate your eye health to determine if the Vivity is appropriate. The goal is always to match the right lens to your eyes and your life.

The Technology Behind X-WAVE™

X-WAVE™ is the patented light-shaping technology inside the Vivity IOL. It represents a completely new way of designing extended depth of focus lenses.

Shapes Light Instead of Splitting It

Two precision-engineered elements on the front surface of the lens gently stretch and redirect light as it passes through. This extends your focus by about 1.5 diopters beyond a standard monofocal lens, creating one continuous zone of clear focus rather than dividing light into separate points the way older multifocal lenses do. This is why the Vivity excels at mid-range distances but may not fully cover fine print at very close range.

Better Contrast Than Diffractive Multifocals

Because the Vivity does not split light, a greater share of the light passing through the lens goes toward forming a clear image compared to diffractive multifocal lenses. While clinical testing shows some contrast reduction versus a standard monofocal lens, the Vivity's contrast performance is much closer to a monofocal than traditional multifocals, which is a meaningful advantage in varied lighting conditions.

Built on the Clareon® Platform for Long-Term Clarity

The Vivity is made with Alcon's Clareon® material, a high-quality acrylic designed for long-term optical clarity with low rates of cloudiness (glistening) in laboratory and clinical testing. The lens filters UV light and includes a blue-light filter, a feature designed to help protect the retina.

What the Clinical Evidence Shows

Published studies from around the world confirm that the Vivity performs well in real patients, not just in controlled trials. Some of the studies cited below received funding from Alcon, the manufacturer of the Vivity lens.

Multi-Center Lens Comparison (Hovanesian et al., 2022)

In this Alcon-funded study of 60 patients, 85 percent of Vivity patients reported no or very little halos and glare, versus 69 percent of PanOptix® trifocal patients. Satisfaction reached 95 percent, and 98 percent said they would recommend the Vivity. However, only 33 percent of Vivity patients achieved complete spectacle independence (never needing glasses), compared to 83 percent of PanOptix patients.

Real-World Patient Results (Giannuzzi et al., 2024)

In a study of 40 patients, 53 percent were completely free of glasses at all distances, and 72.5 percent gave their vision a perfect quality score with zero complaints. No serious complications requiring additional intervention were reported.

Largest Real-World Study (885 Patients, 2025)

The largest real-world study to date followed 885 patients across 8 countries and confirmed excellent distance and mid-range vision, with satisfaction reaching 92 percent among cataract patients. Near vision in the cataract cohort averaged approximately 20/40, with some patients achieving 20/32. These results held true even for patients with mild glaucoma, dry eye, or a history of LASIK.

Individual results may vary. In FDA clinical trials, 1 to 2 percent of patients reported very bothersome visual disturbances, though no patients required lens removal. The data above comes from peer-reviewed studies and FDA trial results. Our cataract surgeons will walk you through the latest research during your consultation.

What to Expect with the Vivity IOL

Most Vivity patients feel comfortable within one to four weeks — generally one of the shorter adjustment periods among premium lenses.

Week 1: Early Adaptation

Because the Vivity stretches and shifts light rather than splitting it into separate focus points, your brain has less to get used to compared to trifocal lenses. Distance vision is usually sharp right away, and mid-range vision improves quickly. Use your vision at all distances every day — reading, working on your computer, and walking around your home. Try to avoid going back to old glasses during this period.

As with any premium lens, allow time for your eyes and brain to adjust before driving at night or in other dimly lit conditions.

Weeks 2 to 4: Fully Adjusted Vision

Most patients feel fully adjusted within one to four weeks. You can expect strong distance vision and excellent mid-range clarity that covers everyday tasks like computer work, reading your dashboard, and shopping. Near vision typically ranges from 20/32 to 20/40, meaning most close-up tasks are comfortable without glasses, though you may want readers for very small print.

Long-Term: Lasting Clarity

The Vivity IOL is designed to last a lifetime. If a thin film develops behind the lens over time (called posterior capsule opacification, or PCO), it can be quickly and painlessly treated with a brief laser procedure in the office. PCO may be noticed sooner with EDOF lenses than with standard monofocal lenses. For very small print or extended reading sessions, many Vivity patients like to keep a pair of light reading glasses handy.

Cost and Insurance Considerations

What Is Covered

The Vivity IOL is a premium lens, which means Medicare or your private insurance typically covers the base cataract surgery itself. The upgrade to the Vivity lens and the extra planning that goes into it are paid out of pocket. Our team goes over all costs and options during your consultation so you know exactly what to expect.

Weighing the Investment

For many patients, choosing a premium lens like the Vivity is a long-term investment in better vision and greater independence from glasses. The combination of extended range, minimal visual side effects, and high patient satisfaction makes the Vivity a strong option for patients who want clear, comfortable vision across their daily activities. You can learn more about typical costs on our cataract surgery cost and insurance page.

Frequently Asked Questions

Common questions about the Vivity IOL answered by our cataract surgery team.

Does Vivity cause fewer halos and glare than multifocal IOLs?
Yes. In clinical studies, 85 percent of Vivity patients reported no or very little halos and glare, versus 69 percent of PanOptix® trifocal patients. Because the Vivity does not split light the way traditional multifocal lenses do, its nighttime side effect profile is much closer to a monofocal lens than other premium options. However, the FDA data shows that 1 to 2 percent of Vivity patients did report very bothersome visual disturbances, so the experience is not identical to a monofocal for every patient.
What range of vision does Vivity provide?
The Vivity gives you strong distance vision, usually around 20/20, and excellent mid-range vision at 20/25 or better. That covers everyday tasks like working on a computer, reading your car dashboard, and shopping. Near vision typically ranges from 20/32 to 20/40 depending on the study and patient population — the FDA trial and some smaller studies report about 20/32, while the larger 885-patient real-world registry found approximately 20/40 for cataract patients. Most close-up tasks are comfortable without glasses, but for long sessions of reading small print, many patients keep a pair of light readers nearby.
How does Vivity compare to PanOptix?
The PanOptix® trifocal IOL gives you sharper close-up vision and a much better chance of never needing glasses at all — in the Hovanesian study, 83 percent of PanOptix patients achieved complete spectacle independence, compared to 33 percent of Vivity patients. The Vivity gives you a wider mid-range and significantly fewer visual side effects like halos (85 percent reported none or minimal vs 69 percent for PanOptix), but close-up vision is not as strong. Both lenses provide similar distance vision. Your choice comes down to whether you would rather have the most glasses freedom possible or the cleanest nighttime vision with the fewest side effects.
Can Vivity be combined with a different lens in the other eye?
Yes. A growing number of surgeons use a "mix-and-match" approach, placing a Vivity in one eye and a PanOptix® trifocal in the other. In early clinical reports presented at professional meetings, mix-and-match patients have shown high rates of spectacle independence and low rates of visual disturbances. This approach can give you the stronger close-up range of a trifocal in one eye while keeping the Vivity's clean nighttime performance in the other. Our cataract surgeons will talk with you about whether this option makes sense for your goals.
Can premium lenses develop clouding or need replacement?
The Vivity IOL is designed to last a lifetime. The most common issue that can develop over time with any lens implant is a thin cloudy film that forms on the membrane behind the lens (called posterior capsule opacification, or PCO). PCO may be noticed sooner with EDOF lenses than with standard monofocal lenses, but if it happens, it is easily fixed with a quick, painless laser procedure in the office that takes just a few minutes. The Clareon® material used in the Vivity is designed for long-term optical clarity with low rates of cloudiness in clinical testing.
I have early glaucoma. Can I still get the Vivity?
In many cases, yes. Because the Vivity does not split light, its contrast sensitivity performance is better than traditional multifocal lenses. However, like all EDOF lenses, it does show some contrast reduction compared to a standard monofocal lens. For patients with mild, stable glaucoma, this tradeoff is generally acceptable, and studies show the Vivity provides noticeably better mid-range and near vision than a standard single-focus lens without adding significant visual side effects. If your glaucoma is more advanced, our surgeons will evaluate your specific situation and may suggest a monofocal lens as a safer choice.
How much does the Vivity IOL cost?
The Vivity is a premium lens, so Medicare or your private insurance typically covers the base cataract surgery, but the upgrade to the Vivity and the extra planning involved are an out-of-pocket cost. Our team walks you through all pricing and options at your consultation so there are no surprises. You can learn more on our cataract surgery cost and insurance page.
Can Vivity correct astigmatism?
Yes. The Vivity IOL Toric version can fix corneal astigmatism at the same time as your cataract surgery. Our surgeons take precise measurements before the procedure to decide whether the toric version is right for you. This can help reduce your need for glasses even if you have moderate astigmatism.
How quickly will I adapt to the Vivity IOL?
Most patients feel comfortable within one to four weeks, which is generally one of the shorter adjustment periods among premium lenses. Because the Vivity stretches and shifts light rather than splitting it into separate focus points, your brain has less to get used to compared to trifocal lenses. See our What to Expect timeline above for a week-by-week breakdown of the adjustment process.
What is the Important Safety Information for the Vivity IOL?
The Clareon® Vivity® Extended Vision Intraocular Lens is indicated for the visual correction of aphakia in adult patients in whom a cataractous lens has been removed. The lens is intended to provide distance, intermediate, and some near visual acuity with increased spectacle independence. Patients should be advised that some reduction in contrast sensitivity may occur compared to a monofocal IOL. As with any premium lens, you should exercise caution when driving at night or in dimly lit conditions while adjusting to the lens. For full prescribing information, visit alcon.com.
