Cataract Surgery
We invested in the ALLY Robotic Cataract Laser System, the most advanced cataract surgery platform available today, because better technology means better outcomes for your eyes. This next-generation system combines robotic laser precision, augmented reality 3D imaging, and adaptive intelligence in a single device, delivering a level of accuracy, speed, and customization that was not possible before.
Why Precision Matters
Modern cataract surgery is not just about removing a cloudy lens. It is a refractive procedure where every fraction of a millimeter affects the clarity of your vision. The opening your surgeon creates in the lens capsule must be perfectly circular, precisely sized, and centered on your visual axis. If it is even slightly off, your new lens implant can shift, tilt, or produce unwanted visual distortions.
This level of precision matters even more with today's premium lens implants. Trifocal, extended-range, and toric lenses are engineered to correct distance, near, and intermediate vision, but they are extremely sensitive to alignment. With every degree of toric lens misalignment, approximately 3% of the corrective power is lost. Traditional manual ink marks used to guide alignment can spread, fade, or introduce errors of 2–5 degrees during surgery.
That is why we invested in the ALLY Robotic Cataract Laser System. It replaces hand-drawn marks and manual incisions with robotic-guided laser precision, iris-registered digital alignment, and augmented reality 3D imaging that scans your eye from six angles. The result is a surgery tailored to your unique anatomy, with the accuracy that premium lenses demand to deliver the vision you are expecting.
Why Patients Choose Us for Cataract Surgery
Every advancement in our approach is designed with one goal: giving you the best possible vision with the most comfortable experience.
Clearer Vision, Less Dependence on Glasses
Premium lens implants can correct distance, mid-range, and near vision, so many patients need glasses far less after surgery.
Shorter, More Comfortable Procedures
In a prospective clinical study, the ALLY system saved approximately 8 minutes of total patient time in the operating room compared to traditional femtosecond laser cataract surgery.
Gentler Surgery for Your Eyes
The laser softens your cataract before removal, using less Ultrasound Energy vs. Manual Surgery. That means a gentler procedure and a more comfortable recovery.
Treatment Customized to YOUR Unique Eye
Advanced 3D imaging scans your eye from six angles, and our system automatically adjusts the surgical plan based on your exact cataract and eye anatomy.
All-in-One Procedure Room
Unlike older laser systems that require moving between rooms, our system performs every step in a single sterile environment. No transfers, no disruptions.
Fellowship-Trained Specialists
Our four cataract surgeons have completed specialized fellowship training and collectively performed thousands of procedures. Your eyes are in experienced hands.
Types of Cataract Surgery
The type of cataract surgery you choose affects how precisely each step is performed. Here is how the three main approaches compare.
| Feature | Manual Cataract Surgery | Basic Laser Cataract Surgery | Robotic Laser Cataract Surgery |
|---|---|---|---|
| Incision Method | Handheld surgical blade | Femtosecond laser | Robotic-guided femtosecond laser |
| Lens Opening | Created by hand | Laser-assisted | Automated with 3D imaging guidance |
| Cataract Removal | Ultrasound only | Laser pre-softening + ultrasound | Adaptive, density-based softening + ultrasound |
| Astigmatism Correction | Manual ink marking | Laser incisions | IntelliAxis iris-registered guidance |
| Imaging | Standard microscope | Basic laser imaging | Augmented Reality 3D imaging from 6 angles |
| Procedure Room | Single room | Typically two rooms | Single sterile room (no transfers) |
| Precision Level | Depends on surgeon skill | Improved consistency | High reproducibility and precision, based on published clinical data |

How the ALLY System Works
From robotic docking to treatment completion, your surgeon maintains full control while the ALLY's intelligent systems guide every step with precision.
ALLY Robotic Cataract Laser System
A personalized, faster, and gentler experience. The ALLY is the first system to combine robotic laser precision with advanced imaging and cataract removal in one compact device. FDA cleared in 2022, it represents the next generation of laser cataract surgery.
Augmented Reality 3D Imaging
Your surgeon sees your eye in incredible detail. The system scans the inside of your eye from six different angles and creates a precise 3D model. This allows your surgeon to plan every step with a level of accuracy that was not possible before.
Adaptive Intelligence
Your surgery is customized to your exact cataract. The system automatically measures your cataract's density and adjusts the laser energy and fragmentation pattern to match. This means less energy used inside your eye and a surgery that is tailored to you.
IntelliAxis Astigmatism Correction
Precise correction so you may need glasses less. Instead of using ink marks that can shift or fade, this technology uses iris registration (a digital map of your unique iris pattern) to permanently mark the exact correction axis on the lens capsule. In studies, 88.46% of patients achieved target astigmatism correction with this approach.
Premium Lens Implants
Choose the vision that fits your lifestyle. We offer a full range of premium intraocular lenses that can correct distance, mid-range, near vision, and astigmatism. Your surgeon will help you find the best match during your consultation.
Premium Lens Options
Premium intraocular lenses (IOLs) go beyond basic cataract removal. They can reduce or even eliminate your need for glasses after surgery. Here are the options we offer.
PanOptix Trifocal IOL
The PanOptix Trifocal IOL provides three distinct focus points (distance, mid-range, and near) designed to reduce or eliminate the need for glasses at virtually every distance. In clinical studies, approximately 80% of PanOptix patients reported complete freedom from glasses for daily activities.
Clareon Vivity Extended Vision IOL
An extended range of vision with excellent clarity and fewer visual side effects. The Vivity uses a smooth, non-diffractive design that stretches your focus rather than splitting light, which means fewer halos and glare compared to traditional multifocal lenses.
TECNIS Symfony OptiBlue IOL
A continuous range of focus from distance to near. The Symfony is designed to provide a smooth, uninterrupted range of clear vision, with technology that also helps reduce the effects of chromatic aberration (color fringing) for sharper, crisper images.
Light Adjustable Lens (LAL)
The only lens that can be fine-tuned AFTER surgery. Once your eye has healed, your doctor uses a special light treatment to adjust the lens power to your exact needs. This means your final prescription is based on how your eye actually heals, not just a prediction.
Every patient's eyes are different. Your surgeon will recommend the best lens option based on your eye health, lifestyle, and vision goals during your consultation.
Research and Results
Clinical studies show meaningful improvements in precision, comfort, and efficiency with the robotic laser approach used at our practice.
The data below comes from a prospective clinical study (Chapman Jr, 2025) comparing the ALLY Robotic Laser System to a traditional femtosecond laser platform, as well as published data from the ALLY manufacturer's clinical program. Every number represents a real benefit for you as a patient.
Individual results may vary. Astigmatism data from published IntelliAxis clinical studies. Time and energy data from Chapman Jr (2025), a prospective study comparing ALLY vs. LenSx workflows (n=23 per group). Your surgeon will discuss what these results may mean for your specific situation.

He offers laser cataract surgery with premium lens implants and is experienced in MIGS (minimally invasive glaucoma surgery), including iStent placement at the time of cataract surgery. If you have both cataracts and glaucoma, Dr. Shakarov can address both in one procedure.
Cataract + Glaucoma Surgery
One Surgery, Two Problems Addressed
If you have both cataracts and glaucoma, you may benefit from a combined procedure. During your cataract surgery, your surgeon can place a tiny device called an iStent (a microscopic stent that helps fluid drain from your eye) to help lower your eye pressure. This is known as MIGS, or minimally invasive glaucoma surgery.
The benefit for you is significant: instead of two separate surgeries and two separate recovery periods, everything is handled in one visit. Many patients who receive a MIGS device during cataract surgery are able to reduce or even eliminate their dependence on daily glaucoma eye drops.
Our glaucoma-fellowship-trained surgeons, Dr. Shields and Dr. Shakarov, have extensive experience performing these combined procedures.
What to Expect
From your first visit to enjoying your new vision, here is what your cataract surgery journey looks like at Greenwich Ophthalmology Associates.
Step 1: Your Consultation
We perform a comprehensive eye exam that includes detailed measurements of your eye, a corneal map, and retinal imaging. We spend time talking with you about how you use your vision every day, your lifestyle, and your goals. Together, we will discuss your lens options and answer any questions you have. This is your opportunity to learn everything you need to feel confident about your decision.
Step 2: Surgery Day
Cataract surgery is a quick outpatient procedure. Most patients are in and out the same day, with the procedure itself typically taking under 30 minutes. Your eye will be numbed with drops so you stay comfortable throughout. Our fellowship-trained surgeons gently remove your cloudy lens and replace it with your new artificial lens, carefully positioning it for the best possible vision.
Step 3: Recovery
Most patients notice improved vision within the first few days. We schedule follow-up visits to make sure your eyes are healing well and your vision is on track. You will use prescription eye drops as directed to help with healing and prevent infection. Most people feel comfortable returning to light daily activities within a day or two.
Step 4: Enjoying Your New Vision
Within a few weeks, most patients are fully healed and enjoying clearer, brighter vision. Many patients find they need glasses far less than before, especially if they chose a premium lens implant. Whether it is reading, driving, working on a computer, or enjoying time with family, you can get back to the activities you love with renewed confidence in your vision.
