What Is Central Serous Retinopathy
Central serous retinopathy, often called CSR or central serous chorioretinopathy, occurs when a small area of the retinal pigment epithelium (RPE) begins to leak. Understanding the basics of this condition helps patients recognize symptoms early and seek timely care.
The retina is the light-sensitive tissue at the back of the eye that converts images into signals your brain can interpret. Directly beneath the retina sits the retinal pigment epithelium, a single layer of cells that nourishes the retina and pumps fluid away from it. When the RPE develops a tiny defect, serous fluid from the choroid (the vascular layer beneath the RPE) seeps through and pools under the central retina, creating a small blister-like detachment.
Most first-time episodes are classified as acute CSR. In acute cases, the fluid pocket typically reabsorbs within three to four months and vision returns to near-normal levels. Chronic CSR is diagnosed when fluid persists beyond this window or when repeated episodes cause lasting changes to the RPE. Over time, chronic CSR can lead to permanent thinning of the retina and measurable vision loss, making early monitoring essential.
Because CSR affects central vision, patients sometimes worry they may have age-related macular degeneration or a macular hole. While these conditions share some symptoms, CSR is driven by RPE leakage rather than drusen buildup or a structural break in the macula. Causes and Risk Factors

- Male sex (men are affected roughly six to eight times more often than women)

Symptoms of Central Serous Retinopathy
CSR usually affects one eye at a time, though both eyes can be involved in some patients. Recognizing these visual changes early allows for prompt evaluation.
The hallmark symptom is a blurred or dim spot in the center of your vision. Straight lines may appear bent or wavy, a distortion known as metamorphopsia. Some patients notice that objects look smaller in the affected eye compared to the unaffected eye, a phenomenon called micropsia. Colors may also appear washed out or slightly different between the two eyes.
In mild cases, symptoms can be easy to overlook, especially if the unaffected eye compensates. Patients sometimes discover the problem only when they cover one eye while reading or looking at a screen. If you notice any unexplained change in central vision, even a mild one, scheduling a retinal evaluation is an important next step.
A sudden shower of new vitreous floaters, flashes of light, or a dark curtain moving across your field of vision are not typical of CSR and may indicate a retinal tear or detachment. These symptoms require immediate evaluation by a retina specialist.
Diagnosis and Treatment Options

Optical coherence tomography (OCT) is the primary tool used to visualize the fluid pocket beneath the retina. In certain cases, indocyanine green angiography helps evaluate the choroidal circulation beneath the retina.
Because most acute episodes resolve spontaneously within a few months, careful observation with periodic OCT scans is often the first approach. During this time, we work with patients to identify and reduce modifiable risk factors such as corticosteroid use and unmanaged stress. If the fluid reabsorbs and vision recovers, no further intervention may be needed.
For chronic or recurrent CSR, photodynamic therapy (PDT) is one of the most effective treatments available. A light-activated medication called verteporfin is injected into a vein and then activated by a low-energy laser directed at the area of leakage. This process helps seal the RPE defect and reduce choroidal hyperpermeability. Reduced-fluence PDT, which uses a lower laser energy setting, has become the preferred approach because it minimizes the risk of collateral retinal damage.
Subthreshold micropulse laser therapy delivers very short pulses of laser energy to stimulate the RPE without causing a visible burn. This technique can encourage fluid reabsorption while preserving surrounding retinal tissue. It is an option for patients whose leakage point is located close to the center of the macula, where traditional laser could cause a permanent blind spot.
Mineralocorticoid receptor antagonists such as eplerenone and spironolactone have shown promise in reducing subretinal fluid in some chronic CSR patients. These medications work by blocking aldosterone receptors, which may play a role in choroidal vascular leakage. Most patients notice gradual improvement over several weeks following photodynamic therapy or micropulse laser treatment. Full visual recovery depends on how long the fluid was present and whether the RPE sustained lasting damage. Early treatment of chronic CSR generally leads to better visual outcomes than waiting for prolonged periods.
CSR and age-related macular degeneration are separate conditions with different underlying causes. However, chronic CSR can produce retinal changes that resemble some features of macular degeneration on imaging.
